Insurance

I have all payment options listed below including not using insurance and using insurance (in and out of network). Also, a form to use to verify your insurance benefits to make that process easier. Please feel free to call or contact me with any questions that you have.

Self-Pay/Private-Pay

If you do not have insurance or choose not to use your insurance you can pay me directly for your sessions. Due to concerns about privacy or their coverage, some people choose not to use their insurance.

If you would like to be a self-pay client, payment is due on/before the beginning of the session. I accept all major credit cards through my electronic health record system and Stripe. I am able to offer a discount to self-pay clients.

Insurance Verification

I have included an insurance verification form that you can use to verify your benefits. Please print this form and call your insurance company. Ask the questions include and write their responses on the lines. This will give you an idea of how much they will cover towards your session.

Insurances Accepted

At this time, Counseling Connections, LLC accepts the following insurance plans for therapy sessions:

  • Full Medicaid (blue card)

  • Presbyterian Centennial Medicaid

  • Presbyterian Commercial

  • Blue Cross Blue Shield Centennial Medicaid

  • Blue Cross Blue Shield Commercial (except Community plans)

  • Molina Centennial Medicaid

  • Molina Commercial (Marketplace Plans)

  • United Healthcare / Optum

Out of Network Plans

For insurances not listed under “Insurance Accepted”, I would most likely be considered an out-of-network provider.

Your insurance may offer benefits that would allow you to pay for your sessions and get reimbursed by your insurance company for part/all of the cost.

To determine your options:

  1. Complete the insurance verification form (see above for more information).

  2. Feel free to call or contact me with any questions, confusions or assistance needed.

  3. If your insurance has out of network benefits, you will pay for your session at the time of service. Then, I can provide you with a “superbill”.

    A superbill is a very detailed receipt that includes all pieces of information that the insurance company needs to process your claim.

  4. You will submit the superbill to the insurance company. They will provide you with reimbursement, according to your benefits.
